LiveBetter gives people who are frail, older or living with dementia the opportunity to stay at home and remain independent for as long as possible. We do this through information, equipment, in-home and community services and by supporting carers.
When you contact LiveBetter, we will assist you to find the most suitable arrangement for your particular circumstances, including referral to the Aged Care Assessment Team if necessary. You will find that our Intake Officers are on your side and will help you through the process.

Consumer Directed Care (CDC)
Home care packages are available under a consumer directed care model. Under this model, a package of funding is allocated to an individual who is then able to direct how that funding is applied to provide services to meet individual needs. There are funding guidelines to ensure the appropriate use of the funding package.

Choice regarding how it is delivered and who delivers it to you;

Collaboration between you and LiveBetter to establish your needs and goals followed by development of a support plan to meet those needs;

Choice regarding the level of involvement you will have in managing your care package;

Monthly Income and Expense statements based on your individualised budget;

and ongoing monitoring and review of your services to ensure they are meeting your needs.
CDC enables you and your carer to tailor the delivery of your services to meet your individual care needs and allows you to exercise a greater degree of choice in the provision of your services. And if your care needs change, re-assessment can be arranged and services reorganised to meet your changing needs.
